Surgery Consent Form

Dear Client,

Your pet is scheduled for an upcoming surgery. To help make the day as smooth and stress-free as possible for both you and your pet, we’ve prepared a few simple forms for you to review and sign in advance. Along with this letter, you’ll find the following documents available on our website:

• Pre-Anesthetic Blood Testing Information Form
• Surgical Information Sheet

Please take a few moments to review all the provided information. If you have any questions, don’t hesitate to contact us at (519) 688-2123 or frontdesk.edwards@gmail.com. On the day of surgery, you’ll be asked to review and sign an Authorization and Estimate Form. We’ll also confirm the best phone number to reach you during your pet’s procedure.

The night before surgery, please withhold all food and treats after 10:00 PM. Water is fine and may be left out for your pet. If your pet is currently receiving any medications, supplements, or injections, please skip the morning dose unless otherwise directed by the veterinarian.

When you arrive on the day of surgery, the doctor will address any remaining questions you may have and collect your completed authorization form.

If you’ve chosen to proceed with any of the recommended bloodwork, samples will be collected and processed prior to surgery, if not already completed in advance. Should any concerns arise, the doctor may contact you using the number you provided.

Following the procedure, we’ll go over your pet’s discharge instructions with you verbally and provide a written copy for your reference. If anything is unclear, please don’t hesitate to ask — either before you leave or once you’re home.
